Temp – Director of Care Management

Position Overview:
  • Interim leadership role overseeing care management across a large healthcare system with multiple hospitals.
  • Responsible for leading approximately 63 care management/social work FTEs and collaborating with physicians, care managers, and social workers.
  • Requires a strategic, innovative, highly collaborative leader who can quickly build relationships and improve operational efficiency.
  • EPIC experience is highly preferred.

Key Responsibilities:
  • Provide leadership and oversight for care management and social work operations.
  • Ensure compliance with federal and state guidelines.
  • Partner with revenue cycle and utilization management teams.
  • Improve discharge planning and address barriers to discharge.
  • Work to reduce readmissions and improve workflow efficiency.
  • Monitor productivity, performance metrics, and team morale.
  • Collaborate across departments and with other market leaders.

Requirements:
  • 36+ months of Care Management experience.
  • 36+ months of interim/progressive leadership experience.
  • Experience supervising at least 10 FTE, PRN, or temporary staff.
  • Experience in a large healthcare system or hospital with 600+ beds.
  • BSN, Bachelor’s degree in Nursing, LISW, or equivalent healthcare management education.
  • Master’s degree in Nursing, Healthcare Administration, Business, or related field preferred.
  • Proficiency with Microsoft tools, including Excel and Power BI.
  • EPIC experience preferred.
  • Stratus finance system experience preferred.
  • Active Iowa or Compact/Multistate license required.
  • BLS certification required.
  • Driver’s license required.
