    Description

    Position Summary: Develops, implements and evaluates diabetes prevention lifestyle curriculum, related classes, demonstrations and other community and family based activities in accordance with Grant guidelines, project protocols, established standards of care and established ADA standards.

    Critical Tasks:

    • Leads the organization and scheduling of Diabetes Prevention Program (DPP) classes, including assignment and location of instructors and other DPP related activities.
    • Oversee and coordinate retention activities such as After-core and support groups.
    • Manage consults with Lifestyle Coaches to ensure contact and follow-up is made to contact referred patients.
    • Lead the screening and recruitment process and intervention activities and ensure adherence to grant protocols, standards of care, and HIPAA requirements.
    • Coordinates the Lifestyle Balance curriculum classes in the SDPI Diabetes Prevention Demonstration project guidelines.
    • Develops, evaluates and modifies the lifestyle curriculum and educational materials in coordination with other grantees, local project team members, and appropriate local community members to incorporate eligibility criteria, evaluation measures, and local cultural values.
    • Works closely with other agencies and departments to integrate educational materials.
    • Works closely with Diabetes Coalition for Native Americans, Tribal, County, State, and GRHC Departments to identify appropriate community resources.
    • Provides diabetes training and education to individual participants, family members, community groups, and other project staff on all aspects of diabetes care.
    • Implements the curriculum, conducting sessions or coordinating and monitoring other staff in conducting sessions according to the agreed upon curriculum, project protocols, and local cultural values.
    • Procures, organizes, evaluates, updates, and develops patient education materials for use by health care professionals, patients and family members, and community-based groups.
    • Develops, implements and evaluates individualized patient educational plan based on assessed need(s) to ensure needs are met.
    • Assists with review/revision of all standards, policy and procedure manuals to ensure ongoing adherence with requirements.
    • Serves as diabetes education resource to other health care providers, patients and patient's families by providing information in response to inquiries regarding services and program availability.
    • Participates in the conceptualization, design, and development of all project protocols, procedures, educational and data collection activities, and infrastructure systems according to grant guidelines, tribal requirements, and local cultural values.
    • Assists in the development of a realistic, measurable work plan of intervention project activities.
    • Assists in preparing progress and data reports; ensuring their accuracy and timeliness in submission.
    • Participates in departmental orientation, on the job training and quality assurance programs/initiatives.
    • Participates in a variety of department and hospital educational programs to maintain current skill and competency levels; identifies and discusses performance or training needs with Supervisor.

    Required Qualifications:

    • Bachelor's degree in Nursing, Nutrition, Public Health, Community Education or closely related field; Master's preferred
    • Current licensure for related field in the State of Arizona preferred
    • Current and valid State of Arizona driver's license
