Lead Case Manager for veterans residing in transitional housing in St. Louis A well-known non-profit in St. Louis is seeking a temporary social worker to work with veterans in a transitional housing setting. This position is full-time, Monday-Friday, normal daytime business hours, temporary 3-6 months with option to extend. Key Responsibilities: - Complete intake/exit assessments, service/care plans, progress notes, case coordination notes, incident reports, and discharge summaries
- Organize veteran classes/group work including life skills, employment skills, and community resource connection
- Guide case management team for all veteran's programming and provide support for productive daily operations
- Govern an organized bookkeeping system, provide reports regarding program activities, progress, and outcomes
