    Summary:With the support of and in partnership with the LEAD team, Case Manager performs a variety of tasks to assist the most vulnerable and chronically homeless individuals, and is responsible for identifying, engaging and providing support to homeless clients referred from the LAPD Pacific division, including aftercare services and follow up support, once housed. The goal of LEAD is to connect with clients experiencing frequent contacts with police due to their homeless status and enroll them in services to prevent entry to the jail and criminal justice system. The Case Manager provides information and support for program members, and assists members in linking to a variety of resources and services as needed. The Senior Case Manager will model competence in wellness, recovery, and coping, and is responsible for maintaining the highest quality services and efficiency of operations.

    Essential Duties and Responsibilities:

    Qualifications:

    High School Diploma or GED or EquivalentValid CA Driver's license, reliable car, auto insurance, and an acceptable driving record.Willing and able to transport Clients in your personal vehicle.Demonstrated knowledge of and experience with the issues faced by the population served (homelessness, mental illness, substance abuse, medical issues, and domestic violence)Willingness to work in a high tolerance Harm Reduction model with clients who have multiple barriersWorking knowledge of LA County agencies serving the homelessExcellent human relations/communication skills; ability to act in professional manner regardless of circumstancesSkilled in non-violent crisis interventionAbility to self-motivate and organize time effectively and efficientlyComputer literate; able to use computerized database system for information managementPassion for working with the population servedAbility to work as part of an interdisciplinary team

    Preferred Qualifications

    Bachelor's degree; or a minimum two (2) years' experience working with homeless individuals, preferably those living with mental illness and/or substance addictions, or an equivalent combination of education and experience.

    Work Environment:

    Combination of field and office environment.Regularly required to sit, stand, bend; occasionally lift or carry up to 35 lbs.On occasion walk or drive to different local sites.Will necessitate working in busy and loud environments.Will be exposed to elements like cold, heat, dust, noise and odor.May need to bend, stoop, twist, and sit throughout the day.
