Job position is available for Full Time, 12:00 PM to 08:30 PM
Yearly Base Salary Pay Range: USD $94972 to $150321

REQUIRED QUALIFICATIONS:
Master's Degree in Social Work from an accredited program.
Upon hire: California Clinical Social Worker License.
2 years of experience in healthcare related field (Acute, Ambulatory, or Post-Acute).
2 years of experience in Acute Care Management (Care Coordination) or successful completion of the Transitions in Practice (TIP) program for Care Managers.
