Position Summary:

The Program Support & Community Services Supervisor, (PSCSS), LCSW is a key member of the Family Bridge Northwest Connecticut leadership and interdisciplinary team. The position supports the development, implementation, daily operations, and sustainability of the Family Bridge Maternal Child Health Home Visiting Program, with primary responsibility for Community Health Worker supervision, family engagement, psychosocial support, community-resource navigation, hospital-based outreach, community integration, and interdisciplinary coordination.

The position provides direct supervision, coaching, and professional support to Family Bridge Community Health Workers and assists in developing effective workflows between Community Health Workers, RN Home Visitors, program support and data staff, healthcare partners, and community organizations.

The Program Support & Community Services Supervisor also performs Program Support Specialist and Community Health Worker functions as needed, including hospital rounding, family engagement, outreach, resource navigation, and connection of families to appropriate community supports.

As a Licensed Clinical Social Worker, the position provides social-work expertise to the interdisciplinary team and may provide psychosocial assessment, behavioral-health support, crisis assessment, counseling, care coordination, and clinical social-work consultation consistent with professional licensure, organizational policy, and scope of practice.

The position works closely with the Director of Family Bridge/RN Supervisor to ensure families receive coordinated, comprehensive, family-centered, culturally responsive, and trauma-informed services.

Community Health Worker Supervision & Leadership

• Provide direct supervision, leadership, coaching, and ongoing support to Family Bridge Community Health Workers.
• Conduct routine individual and team supervision meetings.
• Support CHW orientation, onboarding, training, competency development, and continuing professional development.
• Establish and monitor CHW workflows, assignments, workload, outreach activities, documentation, family follow-up, and program expectations.
• Provide guidance regarding family engagement, social determinants of health, resource navigation, community outreach, and professional boundaries.
• Review CHW documentation and performance indicators for completeness, timeliness, quality, and adherence to program expectations.
• Identify staff-development needs and coordinate appropriate education and support with the Director.
• Participate in CHW performance evaluations and performance-management activities.
• Promote effective interdisciplinary collaboration between Community Health Workers and RN Home Visitors.
• Ensure CHWs practice within established organizational policies, program expectations, and CHW scope and role boundaries.
• Facilitate interdisciplinary case discussions involving complex social, behavioral-health, family, or community-resource needs.

Program Support Specialist / Community Health Worker Functions

The Program Support & Community Services Supervisor may perform Program Support Specialist and Community Health Worker responsibilities based on program needs, including:

• Conduct regular rounding at participating birthing hospitals.
• Maintain a visible, professional Family Bridge presence within hospital maternity and newborn care areas.
• Introduce Family Bridge services to eligible postpartum families during the birth hospitalization.
• Explain the purpose, benefits, voluntary nature, and expectations of Family Bridge services.
• Engage families and identify barriers that may affect participation in Family Bridge.
• Facilitate communication between interested families and the Family Bridge team.
• Support culturally and linguistically appropriate family engagement.
• Participate in hospital-based outreach and program-awareness activities.
• Maintain collaborative working relationships with hospital maternity, newborn, social-work, care-management, and other designated staff.
• Provide hospital-to-community transition support for families participating in Family Bridge.
• Participate in community events, health fairs, outreach initiatives, and other Family Bridge engagement activities.

Licensed Clinical Social Work & Psychosocial Support

• Provide clinical social-work consultation and expertise to the Family Bridge interdisciplinary team.
• Conduct psychosocial assessments within LCSW scope of practice when indicated.
• Assess family strengths, needs, barriers, psychosocial stressors, behavioral-health concerns, safety concerns, and social determinants of health.
• Provide short-term supportive counseling, crisis intervention, and behavioral-health support when appropriate within the Family Bridge model and organizational policies.
• Assist families experiencing complex psychosocial circumstances that require a higher level of social-work expertise.
• Provide consultation to Community Health Workers and RN Home Visitors regarding psychosocial and behavioral-health concerns.
• Recognize and respond appropriately to concerns involving family safety, intimate-partner violence, behavioral health, substance use, housing instability, food insecurity, transportation barriers, or other significant social needs.
• Follow applicable mandated-reporting requirements and organizational policies.
• Support families in accessing behavioral health, social-service, healthcare, and community-based services.
• Promote trauma-informed, strengths-based, culturally responsive, and family-centered approaches throughout the program.
• Maintain appropriate documentation of clinical social-work services in accordance with professional, organizational, regulatory, and program requirements.

Family Support & Community Resource Navigation

• Establish supportive, respectful, family-centered relationships with participating families.
• Identify family needs, strengths, priorities, and barriers related to social determinants of health.
• Provide information and navigation assistance related to healthcare, behavioral health, housing, food security, transportation, utilities, childcare, insurance, parenting support, infant supplies, lactation support, and other identified needs.
• Help families understand available programs, eligibility requirements, and processes for accessing services.
• Advocate for families experiencing barriers to needed services.
• Promote family self-efficacy and independence in navigating healthcare and community-resource systems.
• Support equitable access to Family Bridge and community services.
• Maintain comprehensive knowledge of community resources throughout the Family Bridge service area.

Interdisciplinary Coordination

• Collaborate closely with the Director/RN Supervisor, RN Home Visitors, Community Health Workers, data/program support staff, hospital partners, healthcare providers, and community organizations.
• Participate in interdisciplinary case conferences, staff meetings, program-development meetings, and quality-improvement activities.
• Facilitate communication regarding identified psychosocial needs, resource barriers, family-engagement concerns, and social determinants of health.
• Assist in developing effective workflows between RN Home Visitors and Community Health Workers.
• Recognize circumstances requiring nursing or medical evaluation and promptly communicate those concerns to the RN Home Visitor, Director/RN Supervisor, or other appropriate licensed healthcare professional.
• Support closed-loop communication and coordinated services across healthcare and community partners.
• Maintain clear professional boundaries between social-work, CHW, and nursing scopes of practice.

Community Partnerships, Outreach & Program Integration

• Develop and maintain collaborative relationships with healthcare providers, birthing hospitals, behavioral-health organizations, social-service agencies, community organizations, family-support programs, and other stakeholders.
• Represent Family Bridge at community meetings, coalitions, health fairs, and outreach events as assigned.
• Support development and maintenance of a comprehensive Family Bridge community-resource network.
• Identify gaps in available services and emerging community needs and communicate findings to the Director.
• Assist the Director with community partnership development and program integration.
• Promote awareness of universal maternal-newborn home visiting throughout the Family Bridge service area.
• Support Family Bridge goals related to health equity, accessibility, family engagement, and community integration.

Documentation, Quality & Program Operations

• Ensure timely and accurate documentation of assigned services, family contacts, hospital engagement, community-resource activities, and clinical social-work interventions.
• Monitor CHW documentation for compliance with established program workflows and expectations.
• Collaborate with program data staff to support accurate information necessary for enrollment, tracking, reporting, quality improvement, and program evaluation.
• Participate in quality-improvement initiatives related to family engagement, social needs, resource navigation, outreach, and interdisciplinary care.
• Assist with monitoring program performance indicators related to CHW activities and family engagement.
• Protect family confidentiality and comply with HIPAA, organizational policies, professional standards, and applicable privacy requirements.
• Assist with program implementation, workflow development, staff education, and special projects as assigned by the Director.

Scope of Practice & Clinical Boundaries

The Program Support & Community Services Supervisor functions within the individual''s professional LCSW scope of practice and may also perform appropriate Program Support Specialist and Community Health Worker functions.
