My AmeriCorps - Home Page Lake Area Free Clinic Workflow Improvement
Member activities will improve processes at the Lake Area Free Clinic allowing for increased access to medical and dental care for low income and uninsured populations of Waukesha County. The Member will also have many opportunities to provide direct patient care by assisting the Patient Service Coordinator, Clinic Manager, and other staff and volunteers. The ideal candidate is bilingual (Spanish/English), self-motivated, solution-focused, and aligned with the Lake Area Free Clinic's mission to improve the overall health of those in need. Lake Area Free Clinic (LAFC) is dedicated to improving health outcomes for individuals facing financial barriers to care. Since 2001, the clinic has provided comprehensive medical services to low-income and uninsured residents throughout Waukesha County. Recognizing a critical need for oral healthcare, LAFC expanded its services in 2017 by opening a Dental Clinic, helping uninsured patients and individuals covered by Medicaid access essential dental care. Member Duties : Collaborate with clinic staff and volunteers to develop and implement patient screening processes that assess clinic eligibility, health insurance options, and community resource needs. Serve as the primary advocate for these workflows by training and supporting volunteers. Help connect patients to medical, dental, and behavioral health services while educating them on the benefits of available care. Assist patients in exploring and enrolling in state and federal health insurance programs and support transitions to other healthcare providers when appropriate. Meet with patients to identify barriers to care and connect them with resources that improve access to healthcare and supportive services.
