Requirements

Board Eligible/Board Certified in Family Medicine or Internal Medicine
Strong commitment to patient-centered care
Team-oriented mindset with a willingness to collaborate within a progressive care model
Open to both new graduates and experienced physicians
Physician voice and partnership supported by site-specific medical directors
