Promotes independence by teaching and modeling life skills such as: personal hygiene, nutrition, understanding of mental and physical health and well-being, methods to access assistance, financial management, coping skills, self-esteem and self-reliance. Works as part of the REACH team to ensure services and resources are coordinated among providers as needed to help individuals served live and participate optimally in the community.
