Job Details:

The Integrative Therapist is responsible for the behavioral health integration into primary care by coordinating between Frawley clinic and Bon Secours Medical Group. This position conducts crisis intervention, clinical assessments and evaluations in order to deliver brief treatment services to individuals and their families when needed. In addition, the Integrative Therapist conducts assessments and screenings, treatment interventions, and engages with community resources for the patient's therapeutic process.

Responsibilities:

  • Available to the PCP during working hours in the event of a crisis and to provide a warm handoff.
  • Provides consultation services to the PCP's to enhance their understanding of the patient, and provides support, referrals and develop treatment plans for patients.
  • Evaluate patients for symptoms of mental disorders and behavioral problems, including mental status examination and psychosocial history.
  • Deliver recovery oriented psychosocial rehabilitation to individuals.
  • Serve as a primary therapist to assigned patients.
  • Actively participate in multidisciplinary team processes, including individualized treatment planning, case reviews, behavioral reviews and discharge planning.
  • Documents screenings, evaluation and treatment activity, in connect care.
  • Prepares patient records and required treatment records and maintains required documentation of work activities.
  • Provide training and education on mental health and SUD as needed.
  • Proficient in and utilization of strength-based assessments and diagnosis, CBT, MI and SBIRT and initiate a plan of care.
  • Consults with collaborating psychiatrist as needed.
  • Performs other duties as assigned

Qualifications/Requirements:

Experience: Minimum of 2 years working in a Clinical Behavioral Health setting post master's degree required.

License/Certification: New York State LMSW, LCSW, Licensed Masters Mental Health Counselor, or Limited Permit Mental Health Counselor required.

Education: Master's in Social Work or Mental Health counseling required.

Other: Demonstrate knowledge/competency of DSM 5. Must have excellent communication/interpersonal skills and knowledge of Evidenced based practice modalities.
