Position Summary

Under the direction of the Intake Manager, collaborates with all internal and external customers in an effort to meet the needs of persons seeking Behavioral Medicine Services. The Intake Specialist manages all aspects of the referral/intake process from the initial request through to disposition of the referral.

Essential Duties and Responsibilities

  • Manages all inquiries to the department related to admission referrals from internal and external customers and documents on the designated form.
  • Conducts an initial clinical screening for admission requests and collects preliminary demographics and verifies insurance benefits
  • Provides assistance to the HPH, ER physician in assessment of patients to determine eligibility for admission based on established admission criteria.
  • Provides appropriate outpatient referrals for ER patients when directed by the ER attending.
  • Coordinates all pre-admission activities with the BMU nursing unit for patients being considered for admission, including validation of required legal paperwork, gathering of other pertinent information; e.g., clinical data establishing criteria for admission, physician and nurses reports, medication administration records, lab results.
  • Coordinates all activities for patients accepted for admission, including completion of admission entry paperwork with the patient.
  • Identifies patients’ presenting problems and completes the preliminary treatment plan.
  • Maintains various logs of activities and participates in department/intake performance improvement initiatives.

Required Qualifications

  • Experience in psychiatric triage or patient assessment is essential required

  • Experience with the chronically mentally ill population required

  • bachelor's degree required

Licensure and Certification

  • BLS/CPR required
  • CPI certification required within 90 days of hire

Where a professional license or certification is required, the candidate must maintain the applicable license or certification throughout employment and comply with applicable renewal and reporting requirements.

Schedule

This position follows a full-time schedule. Flexibility may be required based on patient care and operational needs.

Shift Differentials

Shift differentials may apply based on the current Humboldt Park Health policy at the time the hours are worked.

Compensation

Pay Range: $28– $34 per hour

The pay range represents the budgeted compensation range for this position. Final offers may vary based on job-related factors, including qualifications, education, experience, skill set, internal equity, applicable policies, and other factors permitted by law.
