INTAKE COORDINATOR

Castle Partial Program

113 Hospital Drive Willingboro, NJ 08046

Population: Children & Adolescents

Monday through Friday 8A-4P (flexibility required as needed)

Starting at $48,000 ($23 per hour)

MUST have:

A bachelor's degree in psychology, social work or related field.

At least one year experience in the provision of children's mental health services.

This Intake Coordinator will join Castle-Willingboro PHPs steady growth and expansion.

Responsibilities

  • Serves as the first contact for new referrals.
  • Helps to maintain a stable census throughout the program through coordination with members of the treatment team.
  • Coordinate caseloads of clinicians by scheduling intakes and tracking discharge dates.
  • Responsible for screening new referrals for possible admission to the program.
  • Acts as a liaison between referral sources, the patient and family and the program. Follow-up with referral sources regarding intake cancellations/no shows and to give updates on waiting list.
  • Complete discharge follow-up phone calls.
  • Assist families in completing behavior rating scale prior to intake appointment.
  • Assist therapists in case management duties such as scheduling patient appointments, requesting documentation, etc.
  • Complete yearly certification in Handle With Care behavior management system.
  • Act as a community liaison for the program attending events, open houses, provider forums, etc.
  • Assist in tracking referral and discharge statistics.
  • Active participation in coordination of special program events (e.g. Open House, Art Show).
  • Flexibility to offer support to the daily schedule when needed including:
  • Provides direct care and management of patient population.
  • Maintains a safe and therapeutic environment for all patients.
  • Facilitates therapeutic groups that address specific identified treatment issues and reinforces the patient's treatment plan.
  • Provides observational feedback within the multidisciplinary team context.
  • Accurately records daily patient progress in the medical record as well as completing other required paperwork.
  • Rides along with the van driver to transport patients to and from program.

Qualifications

  • Bachelor's degree in psychology, social work or related field.
  • 1 year experience in the provision of children's mental health services.
  • Excellent interpersonal skills.
  • Familiarity with community mental health resources.
  • Excellent organizational and time management skills.
  • Ability to accept supervision and guidance.
  • Demonstrate ability to work independently.
