LSSI is hiring a full-time Intake Coordinator. Responsible for facilitating and coordinating the timely evaluation and acceptance of clients referred for services with varying presenting problems. Manages client caseload linked to medication management services. Serve as the diversion liaison for assigned hospital to ensure continuity of care for clients.
Schedule: 5 days a week, varies.

Essential Functions:
- Complete crisis and initial intake assessments with clients, scheduled and walk-ins.
- Assess patients throughout their episode of care to determine medical necessity and to create an individualized plan of care for the identified diagnosis.
- Provide crisis/linkage case management to ensure continuity of care.
- Provide and coordinate a continuum of treatment services between the ED/SCH and BHAL referrals to meet patient needs.
- Comply with productivity standards.
- Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, other LSSI workforce members, and collaborative organizations and/or individual in a dignified, respectful, honest, and fair manner.
Position Qualifications:
- Bachelors degree in social services or related field required.
- Masters degree preferred.
- Background check clearance required.
- LSW/LCSW/LCPC preferred.
- Minimum of one (1) year experience working with adult clients with mental health diagnosis, conducting counseling, crisis intervention or case management required.
- Proficient in the use of the EHR (Electronic Health Record) system required.
- Documented proof of passing the Illinois Medicaid Comprehensive Assessment of Needs and Strengths (IM+CANS) training and test required.
- Demonstrated ability to communicate in a clear, comprehensible manner, both verbally and in writing. Excellent organization, presentation, and pc/computer skills, including experience using Microsoft Office (Outlook, Teams, Word, Excel, PowerPoint) along with other related software.
