    Intake Coordinator - LPN Home Health

    Now hiring an LPN Intake Coordinator! This is a full-time office-based position with excellent benefits based at our corporate headquarters in Little Rock, AR. This position reports to our Intake Manager. This is a non-patient care office role requiring strong attention to detail and excellent computer & communication skills.

    Duties & Responsibilities:

    • Taking calls from prospective home health patients & referral sources
    • Describing Care IV''s home health service options to referral sources & patients
    • Over the phone, evaluating & determining if patient is appropriate for home health care
    • Verifying their payer source is in network with Care IV
    • Assisting the Intake Manager with orders & other clinical informational assistance
    • Sending & receiving faxes with necessary supporting documentation, signed orders, etc
    • Follow up with physicians, clinics, long-term care facilities, hospitals, etc.
    • Coordinating new patient onboarding with our Marketing Team to ensure satisfactory customer service to the referral sources & our patients
    • Inputting new patient data into the computer system & assisting the RN with tracking & data reporting
    • Some rotating on-call may eventually be required
    • Additional duties as assigned

    Required experience:

    • We are seeking Licensed Practical Nurses whose license is active and in good standing with the Arkansas State Board of Nursing

    • Recent experience working in a clinic or other professional medical office setting

    • Excellent computer skills and a working knowledge of office machinery (computers, multi-line phones, fax machines, email, etc.)

    • Solid clinical nursing knowledge with previous skilled nursing experience

    • Experience with Home Health preferred, but not necessarily required
