    Description

    Empath Home Health, a member of Empath Health, is seeking a Home Health Intake Coordinator in Ocala, FL. The Home Health Intake Coordinator is responsible for efficiently coordinating the referral process, ensuring a seamless review, response, and care coordination for patients referred to the organization.

    Location: Ocala, FL (onsite role)

    Schedule: Full-time; 40 hours

    What Youll Do

    • Serves as the primary point of contact for both internal and external customers, providing exceptional customer service.
    • Receives and meticulously documents patient referrals for home health services, ensuring strict adherence to applicable coverage criteria.
    • Reviews referrals to determine the appropriate care setting and communicate effectively with the care center, Business Development, and referral sources as needed.
    • Gathers essential information to assist the clinical team in scheduling evaluation visits.
    • Communicates pertinent patient information to the care center team through approved processes.
    • Coordinates with other departments for insurance verification and obtains payer authorization as necessary.
    • Professionally communicates and follows up with referral sources regarding required documentation for a comprehensive referral.
    • Meets productivity expectations with quality documentation of complete referrals and all required follow-ups.
    • Coordinates the evaluation/admission process and may assign field staff to conduct initial assessments.
    • Ensures compliance with policies, procedures, and regulatory mandates, including adherence to the terms of the Organizations Compliance Program.
    • Performs other duties as assigned

    What Youll Need

    • High School diploma or GED equivalent.
    • Two (2+) years' proven experience in Home Health referral management or a related field.
    • Clear understanding of medical terminology and basic ICD-10 coding.
    • One (1+) year of supervisory experience or demonstrated ability to manage a team.
    • Proficient in Microsoft suite of products, including Outlook, Word, and Excel.
