Empath Home Health, a member of Empath Health, is seeking a Home Health Intake Coordinator in Ocala, FL. The Home Health Intake Coordinator is responsible for efficiently coordinating the referral process, ensuring a seamless review, response, and care coordination for patients referred to the organization.
Location: Ocala, FL (onsite role)
Schedule: Full-time; 40 hours
What Youll Do
- Serves as the primary point of contact for both internal and external customers, providing exceptional customer service.
- Receives and meticulously documents patient referrals for home health services, ensuring strict adherence to applicable coverage criteria.
- Reviews referrals to determine the appropriate care setting and communicate effectively with the care center, Business Development, and referral sources as needed.
- Gathers essential information to assist the clinical team in scheduling evaluation visits.
- Communicates pertinent patient information to the care center team through approved processes.
- Coordinates with other departments for insurance verification and obtains payer authorization as necessary.
- Professionally communicates and follows up with referral sources regarding required documentation for a comprehensive referral.
- Meets productivity expectations with quality documentation of complete referrals and all required follow-ups.
- Coordinates the evaluation/admission process and may assign field staff to conduct initial assessments.
- Ensures compliance with policies, procedures, and regulatory mandates, including adherence to the terms of the Organizations Compliance Program.
- Performs other duties as assigned

What Youll Need
- High School diploma or GED equivalent.
- Two (2+) years' proven experience in Home Health referral management or a related field.
- Clear understanding of medical terminology and basic ICD-10 coding.
- One (1+) year of supervisory experience or demonstrated ability to manage a team.
- Proficient in Microsoft suite of products, including Outlook, Word, and Excel.
