Job Summary and Qualifications Job Summary The Intake Associate acts as a liaison to ensure a seamless transition from the acute care setting to home healthcare. Key responsibilities include processing referrals and admissions, as well as communicating directly with patients and vendors to ensure customer satisfaction. Responsibilities include, but are not limited to, managing the intake process for Home Health, setting up patient account information, and facilitating face-to-face physician encounter visit. Essential Functions 1. Receives referrals from physicians and facilities and initiates the intake process. 2. Reviews, approves and coordinates all referrals directed toward assigned care team. Manages all assigned intake/admission EMR workflow for Home Health patients (initial referrals, physician verification, etc.) per assigned care team. 3. Provides daily reporting to care team regarding pending admissions, new admission and outstanding (open) items. 4. Enters new patient records ensuring accuracy of all patient demographic information. 5. May perform eligibility/insurance verification. 6. Obtains initial payer source eligibility verification upon intake/referral and coordinates indicated follow-up necessary to adequately communicate the patient's expected out-of-pocket expense for services. 7. Attends regular interdisciplinary team meetings for purposes of identifying new admissions and recertifications. 8. Builds and maintains positive relationships and rapport with colleagues, patients and their family members and community personnel. 9. Adheres to communication guidelines established by HCA Healthcare at Home and local and regional management. 10. Provides professional customer service to colleagues, patients, physicians, and other referral sources. 11. Maintains a safe and clean work area and adheres to facility/corporate safety standards. 12. Adheres to established confidentiality standards as required by HCA Healthcare at Home and HIPAA and patient information privacy requirements. 13. Completes special projects as assigned by the management team. 14. Performs other duties as assigned, to include being cross-trained in the functions of Scheduling, Document Control, and Customer Service and providing support in any of these areas as needed.
