About Our Client: Addison Group is working with a respected and established healthcare organization in the Glendale area, seeking a dedicated Intake Coordinator to join their team. This is a contract-to-hire position with long-term potential, supporting a high-volume home healthcare operation.
Job Description: This role serves as the first point of contact for all incoming home health referrals. You’ll work directly with referral sources, verify insurance, gather critical documentation, and coordinate closely with internal teams to help determine case eligibility and ensure timely onboarding of patients.
Key Responsibilities:
- Manage incoming intake calls and gather essential referral data
- Assemble intake packets and ensure paperwork is complete and accurate
- Verify insurance eligibility, primarily Medicare/Medicaid
- Check internal staff availability to support referral fulfillment
- Coordinate with nursing and scheduling teams regarding case loads
- Meet strict 24–48 hour turnaround times for referral decisions
- Maintain thorough documentation and communicate with stakeholders as needed
Qualifications:
- 1–5+ years of medical administrative or healthcare support experience
- Home health care experience strongly preferred
- Knowledge of insurance verification and eligibility (Medicare/Medicaid focus)
- Proficient with high-volume phone systems and calendar/scheduling tools
- Familiarity with medical terminology and healthcare operations
- Excellent communication skills—both written and verbal
- Detail-oriented, highly organized, and comfortable in a fast-paced environment
- Proactive and solution-driven, with the confidence to ask questions and raise concerns

- Schedule: Monday – Friday, 8:30 AM to 5:00 PM
- Location: On-site role in Glendale, CO
