TITLE: Intake Coordinator
REPORTS TO: VP of Clinical Operations

We are seeking an energetic and results driven Intake Coordinator to join our team. Position Summary
The Intake Coordinator works closely with referral sources, patients, and families to ensure a smooth and timely admissions process. This position combines customer service, intake coordination, and sales support, while giving you exposure to referral source relationships and sales strategy.
Key Responsibilities
- Serve as the first point of contact for inbound referrals via phone, fax, or email.
- Communicate with referral sources (hospitals, physician offices, skilled nursing facilities) to gather necessary information.
- Provide education on hospice services to patients, families, and providers (training provided).
- Enter accurate referral and patient information into the EMR system.
- Coordinate with admissions and clinical teams to ensure timely scheduling of services.
- Support the sales/marketing team by maintaining referral logs, preparing reports, and following up on pending referrals.
- Participate in training, ride-alongs, and mentoring sessions with business development team.
- Develop professional relationship-building skills with the long-term goal of advancing into external sales.
Qualifications
- Bachelor's degree preferred (business, healthcare, communications, or related field) or equivalent experience.
- Customer service or sales experience a plus, but not required—training provided.
- Strong communication, interpersonal, and organizational skills.
- Compassionate, professional, and comfortable working in sensitive healthcare situations.
- Motivated, coachable, and eager to grow into a sales career.
