Job Title: Inpatient Palliative Care Clinical Pharmacy Specialist
Location: Bronx, NY

Job Summary

The Inpatient Palliative Care Clinical Pharmacy Specialist provides comprehensive pharmaceutical care to hospitalized patients with serious illnesses. This role focuses on medication management, symptom control, and supporting patient-centered goals of care while collaborating with interdisciplinary teams. The specialist also contributes to clinical education, protocol development, and safe medication practices.

Key Responsibilities

  1. Serve as a core member of the interdisciplinary palliative care team
  2. Review and evaluate medication histories for appropriateness and safety
  3. Identify and manage drug-related problems, including interactions and adverse effects
  4. Assess patient conditions, symptoms, and care needs
  5. Collaborate with care teams to establish patient-centered treatment goals
  6. Recommend and monitor medication therapies for pain and symptom management
  7. Educate patients and families on medication use and treatment plans
  8. Ensure accurate and timely documentation of patient care activities
  9. Implement strategies to promote safe and effective medication use
  10. Provide guidance on nonstandard dosage forms and administration routes
  11. Address cost considerations including formularies and insurance coverage
  12. Support safe handling of high-risk medications
  13. Review and verify medication orders and ensure proper dispensing
  14. Contribute to development of clinical protocols and care pathways
  15. Participate in clinical education, training, and research initiatives
  16. Ensure compliance with hospital policies and quality standards
  17. Participate in committees and interdisciplinary initiatives

Supervisory Responsibilities

  1. Supervise pharmacy trainees, including pharmacists and students
  2. Support training of interdisciplinary healthcare trainees

Qualifications

  1. Active New York State Pharmacist License (in good standing)
  2. PGY-2 residency, fellowship, or equivalent experience in palliative care, oncology, geriatrics, or hospice preferred
  3. Board certification or relevant clinical certification preferred

Physical Requirements

  1. Light physical activity with occasional lifting up to 20 pounds
