Inpatient Care Coordinator

The Care Coordinator is responsible for providing comprehensive case management services to patients with complex medical conditions and/or psychosocial situations to achieve quality cost-effective and optimal clinical outcomes in both inpatient and outpatient settings. He/she will function as an integral member of a collaborative and interdisciplinary team to assist with identifying high risk patients and improving outcomes.
