    · Provide supervision, support, and monitoring for safety concerns.

    · Provide crisis prevention/intervention services to the people served.

    · Monitor the persons supported for symptoms of decompensation, crisis, and medication side effects, and intervene when necessary.

    · Incorporate the CSSP and CSSP Addendum into daily interaction with the persons served.

    · Assist the persons supported with problem-solving different aspects of independent living.

    · Maintain complete and proper documentation for direct service hours, and fully complete documentation prior to leaving a scheduled shift.

    · Participate in scheduled and spontaneous recreational events (while still following the Enriched Living Transportation Policy).

    · Maintain confidentiality by complying with HIPAA and MN Data Privacy.

    · Adhere to your weekly schedule, arrive on time, etc.

    · Attend mandatory staff meetings, and other meetings requested by PC.

    · Maintain clear, open, and honest communication with program management.

    · Other duties as assigned.
