    Position Summary

    National Home Care Corporation is seeking an experienced and compassionate Registered Nurse to conduct in-home assessments for medically fragile children.  

    The assessment nurse evaluates each child’s medical condition, skilled nursing needs, functional abilities, home environment, caregiver support and safety concerns. The nurse prepares accurate clinical documentation to support the development of an individualized plan of care and the submission of required documentation for service authorization.

    Essential Duties and Responsibilities

    •  Conduct comprehensive nursing assessments in the child’s home. 
    •  Review diagnoses, medical history, medications, physician orders, hospital records and supporting clinical documentation. 
    •  Evaluate the child’s current condition and need for skilled nursing or personal-care services. 
    •  Assess respiratory, neurological, gastrointestinal, nutritional, mobility, behavioral and other relevant clinical needs. 
    •  Evaluate medical equipment and treatments, including feeding tubes, tracheostomies, ventilators, oxygen, suctioning, seizure precautions and medication administration, when applicable. 
    •  Identify safety risks within the home and recommend appropriate interventions. 
    •  Interview parents or caregivers regarding the child’s daily care, routines, limitations and level of assistance required. 
    •  Evaluate caregiver availability, knowledge and ability to safely participate in the child’s care. 
    •  Develop or assist with an individualized plan of care based on the assessment, physician orders and medical necessity. 
    •  Complete assessment forms, nursing notes, medication profiles and other required documentation accurately and within established deadlines. 
    •  Communicate with physicians, families, hospitals, therapists, schools, case managers and agency staff as needed. 
    •  Clarify incomplete or conflicting medical information before submitting the assessment. 
    •  Notify the clinical supervisor of significant changes in condition, safety concerns or suspected abuse, neglect or exploitation. 
    •  Maintain confidentiality and comply with HIPAA
    •  Perform other nursing and documentation duties related to the assessment process. 
    Minimum Qualifications

    •  Current, unrestricted Registered Nurse license in Georgia or active multistate nursing privilege authorizing practice in Georgia. 
    •  Graduate of an accredited nursing program. 
    •  Current CPR/BLS certification. 
    •  At least one year of professional nursing experience. 
    •  Strong clinical assessment and documentation skills. 
    •  Reliable transportation, valid driver’s license and automobile insurance. 
    •  Ability to travel to patients’ homes within the assigned service area. 
    •  Ability to complete documentation electronically and meet required deadlines. 
    •  Ability to successfully complete all required background checks, screenings and credentialing. 
    Preferred Qualifications

    •  Pediatric nursing experience. 
    •  Home-health, private-duty nursing, complex-care or utilization-review experience. 
    •  Experience caring for medically fragile children. 
    •  Experience assessing children with tracheostomies, ventilators, feeding tubes, seizures or other complex medical needs. 
    •  Experience developing pediatric plans of care. 
    •  Knowledge of medical-necessity documentation.

    •  Ability to observe the child, medical equipment and home environment thoroughly.

    Schedule

    This is a field-based position with scheduling determined by assessment demand and family availability. Some travel and occasional evening and weekend availability may be required.
