Schedule: Full-Time Days | Monday-Friday | Occasional weekends as needed

Compensation: $31.25-$46.88/hour (Salary-Based Position)

Key Responsibilities

• Coordinate patient care from admission through discharge
• Assess patient needs and develop individualized discharge plans
• Collaborate with providers, nursing staff, patients, families, and community resources
• Facilitate transitions of care and connect patients with appropriate post-acute services
• Monitor utilization and ensure compliance with regulatory and payer requirements
• Promote quality outcomes, patient safety, and efficient resource management
