We are accredited by the Commission on Accreditation of Rehabilitation Facilities and a member of Southwest Michigan Behavioral Health, Michigan Association of Community Mental Health Boards, and the National Council for Behavioral Health.

Responsibilities:
·        Provides case management services in the community to individuals with intellectual 
         and developmental disabilities and their families 
·        Facilitates linking, advocating, coordinating, and monitoring services; to assist consumers 
         in gaining access to health services, financial assistance, employment, education, social 
         services, and natural supports 
·        Assesses needs, develops, and monitors Person Centered Plans and documents treatment 
 
Qualifications:     
·        Bachelor in Social Work or Master's Degree in an appropriate mental health discipline
·        QIDP, one years’ experience providing treatment to individuals with Intellectual and 
         Developmental Disabilities
·        Must have reliable means of transportation for home visits and maintain Safe Driving Permit
