Obtains report from Nurse being relieved from duty and provides report to Nurse coming on duty; keeps the clinical team and others, as appropriate, informed about the status of patients and related matters; performs or supervises complete, timely and accurate documentation of patient care. Assesses patient’s condition, learning needs and family dynamics based on systematic data collection utilizing assessment skills and tools appropriate to the respective practice area.
