ICM Floating Specialist

Shift: 8:30am to 5:00pm

Pay Rate: Starting at $27/hr and can increase DOE

Purpose:(General description and summary

The ICM Float Specialist provides community-based interventions designed to stabilize and support high-needindividuals enrolled in Intensive Case Management (ICM) services in Multnomah County through an assertive engagement approach. Unlike traditional ICM case managers, the Float Specialist does not carry a designated caseload and instead serves as a flexible team resource to increase participant engagement, continuity of care, and service capacity across the program.

The Float Specialist works collaboratively with ICM case managers, mental health clinicians, and other team members to provideadditionalparticipant contacts, outreach, skill building, resource connection, transportation support, crisis response, and engagement services based on current program needs. The position is intended to increase program capacity and responsiveness by providing enhanced participant support,assistingwith engagement of individuals, and offeringadditionalservice capacity during periods of increased participant need.

The Float Specialist also provides coverage for participants when primary staff are unavailable due to leave, illness, vacancies, training, or other operational needs. Bymaintainingparticipant engagement and supporting continuity of care during staffing disruptions, the position helps reduce service interruptions, mitigate risk, andmaintainfidelity to the ICM model.

Services are delivered from a trauma-informed, person-centered, and equity-focused lens, emphasizing strong advocacy, community integration, and connection to resources and peersupports. The Float Specialist follows procedures, outcome goals, and reporting requirements as outlined by New Narrative's contract with Multnomah CountyandCareOregon. Services are primarily community-based and may include outreach at participant residences, hospitals, shelters, community locations, and appointments, with at least two-thirds of work time spent providing services in the field.

Environment:

This is primarilycommunity basedoutreach position where 60-75%of your time will be in the community in a mixture of direct care, coordination, skills training, transportation, or travel.You will beprovidedtechnology to supportcommunity basedwork. Due to the nature of the work, you will be exposed to various weather conditions and expected tomaintainoutreaching unlessdirected tootherwise by your supervisor. There is an office space at a clinic for administrative tasks and team meetings. This isdirectcare role that requires in person contact with participants.

Accountabilities:(Responsibilities of the job)

Case Management

ICMSpecialistwillwork collaboratively withMental HealthClinicians, Clinical Case managers,andparticipantto create and implement the participant’s individual service plan.TheICMteamprovidescomprehensive wrap around servicesthrough inter-disciplinary care toassistindividuals tomeet their basic needs,promote safety,accomplishgoals,as outlined in thecollaborativetreatment plan,andattainthe highest level of independence possible.These services included coordination of care, skills training, case management and connection to local benefits, coordination with medical providers(and other providers as needed) andassistingparticipantsin meeting basic IADL needs. Number of contacts areweekly and community based in combination with coordination with providers and integrated team.

• Assistparticipantin connecting to resourcesandmaintainingresourcesneeded to achieve goals set inthe individualservice and support plan,including:SSI/SSDI, SNAP, vital documents, ID, etc.

• Advocate for and withparticipant.

• Provide transportationusing company vehicleas needed to/from appointments to ensure connection and access to resources, as well as skills training opportunities in the community, etc.

• Monitor, coordinate, and documentall agency services.

• Develop relationships with providers and stakeholders tofacilitateservice provision

• Have regular, frequent contact with consumersandidentifiedproviders/supports.

• Monitor medication distribution,assistwith prescriber visits.

• Coordinate withexternal providers and natural supports for participant’s wellness,including:PCP/medical,money management services,ENCC, FD, prescribers, other community providers,and intervene as necessary with landlords and other community stakeholders to encourage housing stability.

• Connect participants to cultural resources as applicable 

• Regulardocumentedcoordination with integrated team

Skills Training

• In collaboration with participants,identifyingfunctionalbarriers as well as other skills training needs(ie. ADLs,iADLs), creatingagreed upon plan to achieve goals.

• Addressing other identified skills training needs in collaborationwith other providers(ie. Community integration, interpersonal effectiveness, budgeting, medication management, regulation/coping, safety,etc.)

• Collaborates closely with Mental Health Cliniciansto incorporateappropriate interventionsinto each participant’sservice plan. 

• Utilize MI, assertive engagement,and other therapeutic modalitiesto skillbuild aroundrecovery andsymptom management strategies. 

CrisisPrevention andIntervention

• Work proactively withindividualto avoiduseof crisis services and respond to crisis situations andutilizecritical thinking skills to manage and prioritize needs.

• Situational awareness

• Utilize de-escalation practices and connectparticipantto emergency services as needed.

• Use trauma informedpractices and DEI awareness and sensitivity.

• Coordinate respite referrals andentriesas well asoversee respitestay.

• Complete all required documentation, including IRs, within 24 hours of crisis/incident.

• Monitor hospital contact through case management, care conferences, with heavy involvement in transition planning as well as visitation withparticipantwhile hospitalized.

• Minimize number of days spent using crisis/respite services byremainingclosely involvedintransition planning and proactive approach to crisis prevention.

Administration

• Documentall participantcontactwithin72 hoursof service delivery in Electronic Health Record.

• Utilizefeedback informed treatmentto provide outcome-informed care.

• Ensure ROIs and consents are up to date and obtain new paperwork asindicated.

• Document all meaningful interactions with participantsaccording to policy and Oregon Administrative Rules

• Utilizeoutcomes basedcare tools and review regularly

• Participate in one-on-one supervision bi-weekly to review interventions, documentation, and professional goal setting

• Attend and positivelyparticipatein required meetings, including weekly staff meetings and care coordination meetings with LMPsand other involved providers.

• Take point on delegated administrative tasks assigned by supervisor such as updating spreadsheets, conductingchart reviews for required forms, and tracking due dates for annual assessments and authorizations. 

Team Configuration and Meetings:

• The team will coordinate daily and meets for1-2 hourteam meetingsweeklytostaffcomplexcases and plan services. Meetings are clinically focused andfunction as group supervision. 

• NewNarrative ICMSpecialistwill report to the Intensive ServicesProgram Manager.

Experience/Qualifications:

Aminimum oftwoyears ofexperience in social work or related fieldpreferred.Prior experience with SPMI, houseless population, andcommunity basedservices preferred.Spanish/Englishdesirable.Applicantmustpossessthe skills necessary to work independently.Understanding of psychiatric medications and their use,Administrativerules and other applicable laws and regulations, and thorough understanding of acceptable clinical practices and guidelines.
