JOB SUMMARY: The Specialist is the primary advocate for adults with mental illness and chronic/serious medical diagnoses. These consumers will be referred by the Division of Mental Health Services & from within our own contiuum of housing services to establish independent living situations which provide specialized supports. The Specialist must coordinate each individual's services with the rest of the Supportive Housing Team and participate in the direct provision of these services. DUTIES & RESPONSIBILITIES:
