Bebashi - Transition to Hope, a leading nonprofit organization serving Philadelphia's diverse and underserved communities, is seeking a compassionate, mission-driven Licensed Professional Counselor (LPC) to join our Wellness Center team. This role will report to Director of Clinical Services.

Job Summary:
Bebashi-Transition to Hope seeks a compassionate and driven housing counselor to join our new program, which is dedicated to providing transitional housing and supportive services to people with HIV in Philadelphia.
This role will focus on providing housing counseling, coordinating medical case management (MCM), and facilitating placement in subsidized rental units.
The program aims to retain clients in HIV medical care to achieve viral suppression and secure permanent housing within 48 months, prioritizing those who are homeless or unstably housed.
This role reports to the Supervisor of Housing Medical Case Manager.
