Description

A nonprofit organization in Brooklyn, NY is seeking a Housing Case Manager (HCM) for a field-based role supporting residents through case management, care coordination, and housing stability services delivered in the community and in clients' homes.

About the Opportunity:

  • Shift: Day shift
  • Schedule: Monday to Friday
  • Hours: 9am to 5pm
  • Setting: Hybrid (3 days in the field)

Responsibilities:

  • Provide case management services, supportive counseling, and referrals to a caseload of residents
  • Conduct monthly home visits and maintain ongoing contact with clients in their homes and community settings
  • Support housing stability through advocacy, habitability review, and coordination of services
  • Collaborate with medical providers, behavioral health providers, benefits staff, and other community partners
  • Complete timely documentation, maintain client records, and prepare required reports and service plans

Qualifications:

  • At least 3 years of relevant experience in a social services setting working with individuals who have experienced homelessness, mental health issues, or substance use
  • Bachelor's degree in social work, psychology, counseling, or a related field
  • Prior case management experience with field-based work
  • Experience working with low-income and marginalized communities and individuals with complex medical or behavioral health needs
  • Proficiency in Microsoft Outlook, Word, and Excel.
  • Ability to prioritize, multitask, and manage a caseload independently

Desired Skills:

  • Master of Social Work degree
  • Bilingual (English and Spanish)
  • Experience working in housing services or supportive housing
  • Knowledge of harm reduction practices
  • Experience using EICare or similar electronic records systems
