A nonprofit organization in Brooklyn, NY is seeking a Housing Case Manager (HCM) for a field-based role supporting residents through case management, care coordination, and housing stability services delivered in the community and in clients' homes.
About the Opportunity:
- Shift: Day shift
- Schedule: Monday to Friday
- Hours: 9am to 5pm
- Setting: Hybrid (3 days in the field)
Responsibilities:
- Provide case management services, supportive counseling, and referrals to a caseload of residents
- Conduct monthly home visits and maintain ongoing contact with clients in their homes and community settings
- Support housing stability through advocacy, habitability review, and coordination of services
- Collaborate with medical providers, behavioral health providers, benefits staff, and other community partners
- Complete timely documentation, maintain client records, and prepare required reports and service plans
Qualifications:
- At least 3 years of relevant experience in a social services setting working with individuals who have experienced homelessness, mental health issues, or substance use
- Bachelor's degree in social work, psychology, counseling, or a related field
- Prior case management experience with field-based work
- Experience working with low-income and marginalized communities and individuals with complex medical or behavioral health needs
- Proficiency in Microsoft Outlook, Word, and Excel.
- Ability to prioritize, multitask, and manage a caseload independently
Desired Skills:
- Master of Social Work degree
- Bilingual (English and Spanish)
- Experience working in housing services or supportive housing
- Knowledge of harm reduction practices
- Experience using EICare or similar electronic records systems
