What Youll Do
- Collaborate with hospital teams (case managers, nurses, physicians, discharge planners) to coordinate patient discharges
- Ensure timely delivery, setup, and patient education on respiratory & DME equipment
- Verify insurance coverage and complete referral documentation
- Support emergency equipment requests and troubleshoot issues
- Travel to patient homes for equipment setup and follow-up using a company vehicle
- Participate in quality improvement initiatives and compliance activities
- Provide in-service education and CEU presentations to staff
- Lift and transport equipment up to 65 pounds as needed
What You Need
Required:- Valid drivers license with a clean record
- Strong communication and organization skills
- Ability to lift up to 65 pounds
- Proficiency with Microsoft Outlook, Word, and Excel
Preferred:- 3+ years healthcare or medical equipment experience
- Knowledge of insurance verification and medical billing
- Experience working with hospital teams and patient care coordination

This position plays a critical role in ensuring smooth transitions for patients from hospital to home by coordinating respiratory and DME services within Rochesters hospitals.
