HOSPITAL ACQUIRED CONDITION (HAC) PREVENTION COORDINATOR- LPN

JOB REQUIREMENTS Education Required Required: Graduate of an accredited School of Practical Nursing; High school diploma or GED Preferred: Associate degree in Nursing or a related health field Experience Required: Current, active Arkansas LPN license in good standing (or eligible under the Nurse Licensure Compact); minimum of one year of clinical LPN experience in an acute care/hospital setting Preferred: Prior experience with nursing quality improvement, infection prevention, skin/wound care, or clinical audit/compliance functions Knowledge / Skills Required: Working knowledge of pressure injury prevention and catheter/line-associated infection-prevention best practices; strong attention to detail and ability to perform structured daily audits with accurate documentation; effective written and verbal communication skills to coach and support nursing staff; basic data collection and reporting skills Preferred: Familiarity with Joint Commission National Patient Safety Goals and the CMS Hospital-Acquired Condition (HAC) Reduction Program; experience with electronic health record documentation and quality dashboards; wound care or infection prevention certification Physical This is a safety sensitive position. JOB SUMMARY The Hospital-Acquired Condition (HAC) Prevention Coordinator is a hybrid clinical/quality position, housed within Nursing Administration, that combines direct patient care with frontline surveillance and support for hospital-acquired condition (HAC) prevention. Provides hands-on nursing care within the scope of the Arkansas LPN license; Conducts structured daily audits; including pressure injury prevention, Foley catheter maintenance, central venous line (CVL) maintenance, and peripheral IV maintenance and supports nursing staff in closing identified care gaps. This position helps the organization identify risk at the bedside, reinforce best practices for HAC prevention, and collect the data used to drive unit- and hospital-level quality improvement.
