Hillcrest Health & Living is looking for a compassionate, experienced, and relationship-focused Hospice Social Worker to join our Hillcrest Hospice team.
In this role, you'll help patients and families navigate the emotional, social, practical, and financial challenges that can come with serious illness and end-of-life care. Whether you're helping a family understand their options, connecting them with resources, advocating for a patient's wishes, or simply being present during a difficult moment, your work will make a meaningful difference.

What You'll Do
As a Hospice Social Worker, you'll be an essential member of our interdisciplinary hospice team, providing psychosocial support, education, advocacy, care planning, and resource coordination for patients and families.

The right person will be comfortable navigating sensitive conversations, helping families through uncertainty, and balancing emotional support with practical problem-solving and resource coordination.
