Responsibilities:
- Provide psychosocial support and counseling to patients and families throughout the hospice journey
- Complete psychosocial assessments and document them in the patient's chart
- Regularly assess and reassess the psychosocial, anticipatory grief, and pre-bereavement needs of patients and families
- Carry out the Plan of Care, including social service goals, supportive counseling, problem solving, community referrals, and bereavement care
- Connect families with community resources, including financial programs and eligibility support
- Take part in weekly interdisciplinary team meetings
- Help with funeral arrangements and provide counseling at the time of death when requested
Qualifications:
- Master's degree in Social Work preferred; Bachelor's degree in Social Work required
- 1+ year of social work experience in a healthcare setting or social services agency
- Current Social Worker license (required)
- Hospice experience a plus
- Knowledge of community resources, including state and federal financial programs and eligibility requirements
- Understanding of hospice philosophy and the principles of death and dying
- Strong organizational and communication skills
- Valid driver's license and auto insurance (required)
