Hospice Social Worker

Community Health Network (CHN) has been placing experienced nurses, social workers, and physical therapists in home care, hospice, and case management for over 30 years. As a member of the interdisciplinary team, the Social Worker reports to the Clinical Manager and is responsible for planning and providing supportive services terminally ill patients and families.

Well established hospice agency has a need for an experienced Social Worker (MSW) with Hospice or realted experience to see patients in the Merrimack, Nashua, HamptonNH areas.




As a member of the interdisciplinary team, the Social Worker reports to the Clinical Manager and is responsible for planning and providing supportive services terminally ill patients and families. Must be comfortable with issues relative to end-of life care, death and dying.

• Full-time, Monday-Friday. No weekends or Holidays required.
