SALMON Hospice is seeking a compassionate and experienced Weekend Hospice Registered Nurse (RN) to join our growing hospice team. This position will provide additional weekend clinical support to help manage a busy and expanding patient caseload while ensuring patients and families receive responsive, high-quality hospice care.
The Weekend Hospice RN will work Saturdays and Sundays from 8:00 AM to 4:00 PM, collaborating with the hospice interdisciplinary team to address patient needs, provide scheduled and as-needed visits, and support patients and families throughout the weekend.
Essential Responsibilities:
- Provide skilled nursing visits to hospice patients in their homes and other care settings.
- Assess patients' physical, emotional, and comfort needs and respond appropriately to changes in condition.
- Provide symptom management and comfort-focused nursing care in accordance with the established plan of care.
- Complete scheduled weekend visits and respond to urgent patient needs as assigned.
- Communicate changes in patient condition to the appropriate members of the hospice interdisciplinary team and/or provider.
- Educate and support patients, families, and caregivers regarding medications, symptom management, disease progression, and end-of-life care.
- Coordinate with pharmacies, physicians, facilities, and other healthcare providers as necessary to support continuity of care.
- Complete timely and accurate clinical documentation in accordance with agency policies and regulatory requirements.
- Assist the hospice team with managing increased weekend caseload needs and ensuring continuity of care.
- Participate in care coordination and handoff communication with weekday and on-call team members.
- Maintain compliance with hospice policies, procedures, and applicable state and federal regulations.
Qualifications:
- Current, unrestricted Registered Nurse (RN) license in Massachusetts.
- Minimum of one year of professional nursing experience; hospice, home health, palliative care, or end-of-life care experience strongly preferred.
- Strong clinical assessment and critical-thinking skills.
- Compassionate approach to patients and families experiencing serious illness and end-of-life care.
- Ability to work independently in a community-based setting while collaborating effectively with an interdisciplinary team.
- Strong communication, organization, and documentation skills.
- Valid driver's license and reliable transportation required for community-based visits.
