What You'll Do
- Develop and manage individualized hospice plans of care for your assigned patients.
- Complete comprehensive nursing assessments and ongoing case management.
- Monitor pain, symptoms, and changes in condition while promoting comfort, dignity, and quality of life.
- Serve as the primary RN and trusted clinical resource for patients, families, and caregivers.
- Educate patients and loved ones throughout the hospice journey with compassion and confidence.
- Coordinate care with physicians, nurse practitioners, social workers, chaplains, hospice aides, therapists, and other interdisciplinary team members.
- Provide care in private homes, assisted living communities, and skilled nursing facilities.
- Maintain timely, accurate EMR documentation while ensuring compliance with Medicare hospice regulations.
What We're Looking For
- Active Minnesota Registered Nurse (RN) license.
- Experience in hospice preferred, or a strong clinical background in oncology, palliative care, ICU, home health, or chronic illness management.
- Strong assessment skills, sound clinical judgment, and the ability to work independently.
- Excellent communication, organization, and critical thinking skills.
- A deep commitment to dignity, autonomy, and relationship-centered care.
- Reliable transportation and willingness to travel throughout the service area.
