We have Full Time positions in Central and Southern Orange County. Hospice RN Case Manager Responsibilities: * Perform RN duties on hospice team.  * Serve as a RN Case Manager for the hospice Interdisciplinary Team* Assess for signs and symptoms of discomfort including presence of pain and takes prompt and appropriate action* Establishes plan of care while collaborating and communicating with the Interdisciplinary Team including physician when appropriate* Coordinates all hospice services provided to patients/families in all settings to ensure quality of careIdeal Hospice RN Case Manager Candidate Background: * Registered Nurse with valid CA state license* Minimum one year of hospice RN experience* Strong background in medical/surgical, ICU/CCU, or ER is helpful* Valid driver’s license, auto insurance, reliable transportation and willingness to drive to patients place of residence* Must have excellent interpersonal skills and a passion for hospice* Understanding of hospice philosophy and needs of the terminally ill* Self-directed with ability to work with little supervision* Current Physical and TB test resultsHospice RN Case Manager Benefits:
