Reporting to the Managing Director – Hospice, the RN
Case Manager, provides skilled nursing care to hospice patients in their homes
while coordinating individualized care plans with patients, families, and the
interdisciplinary team (IDT). You'll serve as a trusted advocate, ensuring
compassionate, high-quality care throughout the hospice journey.

What You’ll Do
Key Responsibilities
- Perform comprehensive patient
assessments and develop individualized care plans
- Deliver skilled nursing care in
patients homes in accordance with hospice standards
- Coordinate care with patients,
families, physicians, and the IDT
- Supervise Hospice Aides and LVNs to
ensure continuity of care
- Maintain
accurate, compliant clinical documentation
- Complete patient admissions and
participate in PRN on-call rotation
- Manage patient schedules and
prioritize changing care needs
- Promote infection control, patient
advocacy, and agency values
- Assist with training and mentoring
new clinical staff as needed
- Commit to ongoing professional growth and clinical
excellence through continuous education
What We Are Looking For
Qualifications
- Graduate of an accredited Nursing
program (Diploma, ADN, or BSN)
- Active Texas RN license required; Current
CPR certification
- 1–3 years of clinical nursing
experience required; Hospice, ER, or OR experience preferred
- Reliable transportation, valid
Texas driver's license, and current auto insurance
- Must pass background check and
provide required TB and Hepatitis documentation
Additional Requirements
- Ability to stand, walk, safely move
patients, lift up to 50 lbs. and travel locally
- Ability to work effectively in
high-pressure, fast-paced environments
- Strong clinical judgment and quick
decision-making skills
- Ability to meet the individualized
psychosocial needs of patients and families
- Excellent hand-eye coordination and
manual dexterity
