Hospice RN Case Manager | $85K+ | Outstanding benefits + No Weekends

We are seeking a compassionate Registered Nurse (RN) Case Manager to join our Hospice team serving patients throughout Denton County, TX. In this role, you will provide skilled nursing care to hospice patients in their homes, manage symptoms, coordinate care with the interdisciplinary team, and support patients and families through the end-of-life journey.

Schedule

• Monday–Friday
• Full-Time
• Travel within assigned territory

Responsibilities

• Provide skilled nursing care based on individualized care plans.
• Perform comprehensive patient assessments and monitor changes in condition.
• Administer medications and treatments as ordered.
• Educate patients and families on hospice philosophy and symptom management.
• Coordinate care with physicians and the interdisciplinary hospice team.
• Maintain accurate EMR documentation.
• Respond to urgent patient needs and provide crisis intervention.
• Ensure compliance with hospice regulations and standards.

Required Qualifications

• Active Texas RN License.
• Minimum 1 year of Hospice RN experience (Required).
• Minimum 1 year of Case Management experience (Required).
• Reliable transportation, valid driver's license, and current auto insurance.
• Strong communication, assessment, and documentation skills.

Pay and Benefits
