About the Role:
Solara Hospice LLC is looking for a compassionate and dedicated Hospice RN to join our team in Braintree, MA. In this meaningful role, you'll provide expert end-of-life care and emotional support to patients and their families during life's most important moments. If you're a skilled nurse with a heart for hospice, we'd love to have you on our team.



Responsibilities:
  • Conduct comprehensive patient assessments and develop individualized plans of care for hospice patients
  • Provide skilled nursing visits in home, facility, and residential settings across the Braintree, MA area
  • Manage pain and symptom control using evidence-based palliative care practices
  • Educate and support patients, families, and caregivers on disease progression and end-of-life care
  • Collaborate with an interdisciplinary team including physicians, social workers, chaplains, and aides
  • Document clinical findings, care plans, and patient updates in accordance with Medicare/Medicaid hospice guidelines
  • Provide on-call nursing support and respond to urgent patient needs as needed
Requirements:
  • Active RN license in the state of Massachusetts (required)
  • Minimum 1-2 years of nursing experience; hospice, palliative care, or home health experience preferred
  • Strong knowledge of pain management, symptom control, and end-of-life care protocols
  • Excellent communication and interpersonal skills with patients, families, and care teams
  • Ability to work independently, manage a caseload, and prioritize in a fast-paced environment
  • Valid driver's license and reliable transportation for field-based visits
  • Proficiency with electronic medical records (EMR) documentation
