    Qualifications

    Required

    • Active Registered Nurse (RN) license in the State of Connecticut
    • Current BLS certification
    • Minimum of 1 year Hospice nursing experience

    Preferred

    • Home health experience
    • Strong knowledge of symptom management and end-of-life care

    What You’ll Do

    • Perform comprehensive nursing assessments for hospice patients
    • Develop and implement individualized care plans in collaboration with the interdisciplinary team
    • Provide symptom management and pain control according to physician orders
    • Educate patients and families about hospice care, medications, and comfort measures
    • Provide emotional support and guidance to patients and caregivers
    • Coordinate care with physicians, social workers, chaplains, and other healthcare professionals
    • Monitor patient condition and respond to changes in health status
    • Maintain accurate and timely clinical documentation
    • Participate in interdisciplinary team meetings and care planning discussions

    Compensation

    Salary: $100,000 – $115,000 per year
