Key Responsibilities:
Provide skilled nursing care to hospice patients, focusing on pain management, symptom relief, and emotional support.
Manage a caseload of patients, develop personalized care plans, and coordinate care with the hospice team.
Educate patients and families about the hospice process, end-of-life care, and coping strategies.
Assess patient needs regularly, report changes in condition, and adjust care plans accordingly.
Offer emotional support to families and provide guidance throughout the hospice journey.

At the heart of hospice is compassionate, patient-centered care—and our Registered Nurse (RN) Case Managers make that possible every day.
We are seeking a dedicated and experienced Hospice RN Case Manager to join our team serving Forsyth County and the surrounding communities. In this role, you will provide expert clinical care, coordinate services across the interdisciplinary team, and build meaningful relationships with patients and families during one of life's most important journeys.
As the primary nurse managing your patient caseload, you will perform comprehensive assessments, develop individualized plans of care, manage symptoms, provide education and emotional support, and advocate for each patient's comfort, dignity, and quality of life. This is an opportunity to practice nursing with autonomy while making a lasting difference in the lives of patients and their loved ones.

Knowledge, Skills & Abilities
Physical Requirements
Ability to stand, walk, bend, reach, lift, and assist patients as needed throughout the workday.
Ability to travel throughout the assigned service area to provide in-home patient care.
Visual acuity and observational skills necessary to perform comprehensive patient assessments.
Work Environment
Frequent travel between patient homes and healthcare facilities within the assigned territory.
Exposure to infectious diseases and other healthcare-related hazards consistent with patient care.
Work is performed in a variety of home and community settings.
Schedule
Full-Time
Monday–Friday
Participation in an on-call rotation, weekends, and holidays as assigned.
