We are seeking a compassionate and dependable PRN Licensed Vocational Nurse to provide skilled, comfort-focused care to hospice patients in McKinney and Forney.


This field-based position is ideal for an LVN who is passionate about end-of-life care, values flexibility, and wants to make a meaningful difference for patients and families. Applicants may indicate their preferred service territory when applying. Patient visits will be assigned according to nurse availability, geographic location, patient census, and agency needs.


Local travel within the assigned service area is required.

Key Responsibilities


Skilled Nursing Care

• Complete skilled nursing visits according to the patient’s individualized Plan of Care and under the direction of the RN Case Manager.
• Assess and monitor patients’ physical condition, symptoms, comfort, and response to care.
• Provide comfort-focused nursing interventions within the LVN scope of practice.
• Administer medications and treatments as ordered and permitted by applicable nursing standards, agency policy, and the patient’s Plan of Care.
• Assist with pain and symptom management interventions as directed.
• Follow infection-control, medication-safety, and patient-safety procedures during every visit.

Observation and Clinical Communication

• Recognize and promptly report changes in a patient’s condition to the RN Case Manager and physician or other appropriate clinical provider.
• Communicate concerns involving pain, uncontrolled symptoms, medication effectiveness, skin integrity, safety, caregiver ability, or emotional distress.
• Notify the RN Case Manager of any patient or family needs that may require a reassessment or change to the Plan of Care.
• Provide clear and timely updates to promote continuity of care.

Patient and Family Education

• Educate patients and caregivers regarding medications, treatments, symptom management, comfort measures, equipment, and basic caregiving techniques.
• Reinforce instructions provided by the RN Case Manager and interdisciplinary team.
• Help patients and families understand when and how to contact hospice regarding changes in condition or urgent concerns.
• Provide reassurance, compassionate support, and respectful guidance throughout the hospice journey.

Interdisciplinary Collaboration

• Collaborate with RN Case Managers, physicians, social workers, chaplains, hospice aides, facility staff, and other members of the interdisciplinary hospice team.
• Follow the established Plan of Care and communicate any concerns or recommended changes to the supervising RN.
• Participate in care coordination and team activities as assigned.
• Support a patient-centered approach that respects each patient’s goals, preferences, beliefs, and end-of-life wishes.

Documentation and Compliance

• Complete all nursing visit notes and required documentation accurately and within agency-established timeframes.
• Document the patient’s condition, skilled services provided, interventions, response to care, education, communication, and follow-up needs.
• Maintain accurate and professional records in the agency’s electronic medical record system.
• Protect patient confidentiality and comply with HIPAA requirements.
• Follow applicable hospice regulations, professional nursing standards, and Millenia Hospice policies and procedures.
• Perform duties within the Texas LVN scope of practice and under appropriate RN supervision.

Required Qualifications

• Current and active Licensed Vocational Nurse license in the State of Texas.
• Current CPR certification.
• Valid driver’s license and reliable transportation.
• Ability and willingness to travel throughout the assigned service territory.
• Strong clinical observation, communication, and organizational skills.
• Ability to work independently while following the direction of the RN Case Manager.
• Ability to manage time effectively and complete assigned visits and documentation promptly.
• Compassionate, dependable, and patient-centered approach to care.
• Ability to successfully complete required background checks, license verification, and onboarding requirements.
• Commitment to maintaining confidentiality and complying with HIPAA and agency policies.

Preferred Qualifications

• Previous hospice or home health nursing experience.
• Experience providing care in patients’ homes or residential facilities.
• Familiarity with end-of-life care, pain and symptom management, and caregiver education.
• Experience using an electronic medical record system.
