Hospice Nurse Urgent NEED

• Coordinates hospice care by workingwith other team members as well as with the patients/family members to ensureneeds are met.
• Evaluate the effectiveness ofhospice services to the patient and family on an ongoing basis.
• Perform evaluations for hospice carealong with informational visits as needed.
• Utilize the nursing process toprovide professional nursing care.
• Submit patient visit scheduleweekly.
• Provide guidance and supervision tothe LPN and supervises the LPN a minimum of every 30 days.
• Prepare the care plan for theHospice Aide/Home Attendant.

Requirements

• Must be a graduate from an accredited School of Nursing
• Must be licensed in the State of Virginia as a RegisteredNurse
•  Hospice/Home care experience preferred (extensive training provided)
