The Community Hospice is seeking a full-time Hospice Nurse Practitioner. This is an exciting opportunity to join a respected and compassionate team of caregivers as it continues to provide service to the Capital District and surrounding counties.
The Community Hospice is a not-for-profit 501(c)(3) organization that serves nine counties in the Capital Region of upstate New York through three regional offices and is accredited by the Community Health Accreditation Program (CHAP). We are part of St. Peter's Health Partners and are privileged to work with every hospital in the region to provide hospice services. We also collaborate with 43 skilled nursing facilities and many assistive living/adult homes across our service area. The majority of our patients are served where they prefer to be at end of life to maximize quality of life - their home. We have an average daily census of over 700 patients. Our service area covers: Albany, Schenectady, Rensselaer, Saratoga, Columbia, Greene, Schoharie, Washington and Montgomery Counties.
Position Details:
- Provide oversight and medical consultation for hospice patients including establishing appropriate pain and symptom management, provision of Advance Care Planning discussions, and assisting with discharge dispositions as appropriate.
- Monday-Friday schedule (8am-4pm) with rotating weekend daytime coverage
- Conduct hospice visits to aide in symptom management, establish goals of care, and complete Medicare face-to-face requirements
- Evaluate patients end of life needs and guide development of individualized hospice plans of care
- Serve as primary care provider for hospice patients without an active PCP
- Lead advance care planning discussions and assist with discharge planning when appropriate
- Collaborate closely with interdisciplinary teams including nursing, social work, and chaplains

Requirements:
- Board Certified Nurse Practitioner (FNP, AGPCNP, or AGACNP)
- New York State license and DEA certification
- Experience in hospice, palliative care, geriatrics, or related field preferred
Recruitment Package:

- Compensation range: $138,729-$160,926
- The posted salary reflects the starting range of total compensation and does not include productivity bonuses, incentives related to quality and performance, extra shift incentives, or other forms of cash compensation as applicable to the position
