    Description

    Physician – Hospice Medical Director / Hospice Physician

    Location: Remote: Position Type: Part-Time | Flexible Schedule Practice Setting: Hospice Care

    Overview

    This role offers physicians a flexible leadership opportunity to provide compassionate, high‑quality end‑of‑life care within their local communities. Physicians in this role partner closely with experienced interdisciplinary teams and play a key part in ensuring patients and families receive exceptional hospice support. This position is ideal for physicians seeking meaningful impact while maintaining a schedule that complements their current practice.

    Key Responsibilities

    • Provide medical oversight for hospice patients within the assigned service area
    • Determine and certify hospice eligibility
    • Lead and participate in interdisciplinary team (IDT) meetings
    • Support symptom management, medication oversight, and care planning
    • Complete required physician documentation, including certifications and death certificates
    • Serve as a clinical resource for nurses, care teams, and referral partners
    • Build and maintain relationships with community physicians, hospitals, and referral sources

    Required Qualifications

    Experience

    • 1+ year of Physician Informatics experience
    • 1+ year of experience in ANY of the following:

      • Primary Care (PCP)
      • Hospice
      • Related chronic or end‑of‑life care settings

    Licensure & Credentials

    • Active, unrestricted MD or DO license in the state of practice
    • Board certification or board eligibility in a relevant specialty preferred (Family Medicine, Internal Medicine, Palliative Care, etc.)

    Schedule & Shift Details

    • Flexible schedule designed to complement the physician's current practice
    • Primarily remote work
    • Occasional in‑person interdisciplinary team meetings, community outreach, and leadership activities within the assigned market

    Travel Requirements

    • Occasional local travel for in‑person meetings, community outreach, and market‑based leadership activities
    • Travel limited to the physician's assigned local market
