Salary Range: $45.26-$67.90, DOE

Shift Differentials:

  • $2.20 extra for working weekends


Position Summary

The Asante Hospice Liaison evaluates and facilitates Hospice referrals obtained from the Asante Hospitals, APP clinics and community referral sources. This RN evaluates for hospice appropriateness, utilizing criteria metrics outlined by CMS, clinical records, appropriate lab values, patients functional status as well as patient/family goals for end of life care. The Liaison may also be responsible for coordination and facilitation with the hospice PBM, Community and Facility pharmacies and patient care facilities to ensures medication compliance, formulary development, and authorization of prescribed medications for hospice payment and clinical nurse education.

Qualifications

Education

  • Bachelor's in nursing (BSN) preferred

Experience

  • 2 years clinical hospital experience as an RN and 1 year current Hospice, Palliative care, or Home Care experience is required

License/Certification

  • Registered Nurse (RN) licensed by the Oregon State Board of Nursing required upon start
  • Basic Life Support for the Healthcare Provider (BLS) certified through the American Heart Association (AHA) required upon start
  • Possession of a valid Oregon Driver's License with proof of safe driving record for the past 10 years and proof of insurance is required
