Hospice Liaison / Business DevelopmentHorizon Hospice of ColoradoLocation: Greater Denver Metro and surrounding communities, field-basedJob Type: Full-time, exemptSchedule: Monday-Friday, with occasional evening or weekend community events as neededHorizon Hospice of Colorado is seeking an experienced Hospice Liaison / Business Development professional to build and grow referral relationships throughout the Greater Denver Metro area. This is a field-based growth role, not an intake or referral-processing position.The Community Liaison will develop new referral sources, strengthen existing accounts, and generate qualified hospice referrals through consistent in-person outreach to hospitals, physician practices, skilled nursing facilities, assisted living and memory care communities, case managers, social workers, and other healthcare partners.Candidates with direct hospice liaison/business development experience and established professional relationships within the Denver-area healthcare community will receive priority consideration. Strong home health or post-acute healthcare sales candidates may also be considered if they can demonstrate successful territory development and measurable referral growth.Compensation and Incentive Opportunity$70,000-$82,000 base salary, plus quarterly performance incentive. Target total annual cash compensation is approximately $85,000-$100,000. Mileage reimbursement is provided separately for approved business travel.Incentive bonus eligibility is subject to plan terms, verified performance, documentation, compliance review, and company approval. This is not a commission or per-referral payment.Key ResponsibilitiesDevelop new referral relationships with hospitals, physicians, discharge planners, skilled nursing facilities, assisted living and memory care communities, care managers, eldercare professionals, and community partners.Conduct regular field-based prospecting, account visits, education, in-services, presentations, and follow-up to develop new community referral sources.Identify opportunities within the assigned territory and create strategies to convert prospective accounts into active referral relationships.Educate referral partners, patients, and families about hospice philosophy, palliative care, eligibility basics, and Horizon's referral process.Coordinate timely referral handoff to admissions and clinical teams while honoring patient choice and compliance standards.Track outreach activity, referral development, qualified referrals, admission outcomes, conversion, and follow-up using Horizon's approved tracking tools.Support existing affiliate and community partnerships while maintaining a strong focus on external growth.Attend community events and represent Horizon Hospice of Colorado professionally.QualificationsMinimum one year of outbound, field-based healthcare business development, physician outreach, community liaison, or healthcare sales experience with personal responsibility for developing referral sources or generating new business.Experience limited primarily to receiving, processing, coordinating, or managing incoming referrals does not meet the business development experience requirement for this position.Hospice business development or liaison experience strongly preferred.Demonstrated ability to prospect for new accounts, develop professional relationships, follow up consistently, and produce measurable referral or business growth.Existing professional relationships within the Greater Denver Metro healthcare, senior living, hospital, physician, or post-acute community strongly preferred.Strong communication, presentation, follow-up, organization, and relationship-building skills.Comfortable using Excel, Google Sheets, or other approved tracking tools.Valid driver's license, reliable transportation, current auto insurance, and ability to travel locally.BenefitsEligible full-time employees are offered benefits that may include medical, dental, vision, paid time off, paid holidays, mileage reimbursement, professional development and training, and eligibility for the quarterly incentive bonus plan.
