Pride Health is hiring a Hospice Home Health RN for one of our clients located in PA! (Contract)

 

Schedule: Mon-Fri 8A-4:30pm, (On Call: 2 nights on call, Mon-Thurs)

Contract: 3 months

Pay Rate: 50-55/hour

Coverage Areas: CAMBRIA AND SOMERSET COUNTIES

Productivity expectations: 5-6 PATIENTS/DAY

Mileage Reimbursement: Yes

 

Duties and Responsibilities

  • Coordinate end-of-life care for terminally ill patients, acting as the primary liaison between families, physicians, and the hospice care team which includes the hospice medical director.
  • Assess patient needs, manage pain and symptoms, and oversee the execution of personalized care plans to ensure comfort and dignity.
  • Manage a designated patient caseload, conduct initial and ongoing physical, psychosocial, and spiritual assessments of patients in their homes or residential facilities.
  • Educate caregivers on patient care, disease progression, and the grieving process.
  • Maintain accurate, compliant electronic medical records (EMR) and update physicians on patient status changes.
  • Participate in regular IDT meetings to review care plans and adjust interventions as the patient's condition changes.

 

Requirements and Qualifications

  • Active RN License
  • BLS Certification
  • Hospice experience
