Position Summary
TheClinical Manageroversees the coordination, quality, and delivery of hospice patient care services. This role ensures clinical excellence, supervises interdisciplinary team members, and supports clinicians in providing outstanding patient-centered care.
Pay Range:$140,000 – $150,000 per yearLocation: Concord office +in the field as neededJob Type:Full-time
Key Responsibilities
Receive, review, and evaluate patient case referrals todeterminehospice needs, including conducting home visits.
Assignappropriate hospicepersonnel to each case and ensure propercarecoordination.
Collaborate with the Medical Directorregardinghospice eligibility and patient needs.
Perform ongoing case evaluations through home visits, interdisciplinary meetings, and record review.
Guide and mentor clinicians to promotehighquality, compliant hospice services.
Assistclinicians in developing care plans,establishinggoals, and prioritizing interventions.
Lead andparticipatein interdisciplinary team case conferences.
Conduct quarterly clinical record reviews and communicate findings to leadership.
Support recruitment, interviewing, and onboarding of new clinical staff.
Assistwith planning and delivering continuing education, staff training, and in-service programs.
Support policy and procedure development and implementation.
Ensure compliance with professional standards, hospice regulations, and organizational policies.
Assess community health needs and support program development to meet those needs.
Qualifications
Current California Registered Nurse (RN) License.
2+ years of hospice experience.
2+ years of management or supervisory experiencestronglypreferred.
Strong clinical assessment, communication, and leadership skills.
Comprehensive understanding of hospice philosophy, end-of-life care, and pain/symptom management.
Valid driver’s license, reliable insured vehicle, and current CPR certification.
Ability to work collaboratively with interdisciplinary teams and support clinicians in the field.
